Notes: Enter Ca carbonate as mg of salt, not of calcium. Enter Auryxia and Velphoro as mg iron. Usual pill sizes in mg are 667 for calcium acetate (Phoslo); 500, 750 or 1000 for Fosrenol (enter as weight of lanthanum ion); 800 and 2400 (packets) for sevelamer; 210 (as iron) for Auryxia; and 500 (as iron) for Velphoro. Typical PBED requirements are ~6 g/day in anuric men and ~5.5 g/day in anuric women, and are about 25% lower in patients who still make some urine. See Table 5 in Am J Nephrol. 2019;49:368-376 for conversion factors (PBED/g binder). For sevelamer conversion factor we used y= -0.0167x + 0.8667 where x= g sevelamer per day. Ca carbonate is 40% calcium by weight, and Ca acetate is 25%.
